TRANSITIONS FLEXI TRIP PROMO

MECHANICS:

1. Promo period is from December 1, 2022 to January 31, 2023.

2. Customers can earn raffle tickets with every purchase of:

  • One pair of any single vision Transitions lenses = 1 Raffle Ticket
  • One pair of any progressive Transitions lenses = 2 Raffle Tickets

3. This promo is open to all patients/customers of Ideal Vision Center who are at least 18 years old by December 1, 2022, and are currently residing in the Philippines.

4. Patients/customers must completely fill out the raffle ticket with the following details: Complete Name, Address, Contact No., Birthday, Email, Branch, Name of Doctor, Rx Number and Signature and drop at the drop box located in all participating IVC branches nationwide.

5. Patients who are under 18 years old may be represented by their parents or legal guardian.

6. Prizes:

  • Grand Prize: One (1) Winner of 3D/2N FlexiTrip to Boracay for 2
  • Consolation Prizes:
    • One (1) Winner of 3D/2N FlexiTrip to Bohol for 2
    • One (1) Winner of 3D/2N FlexiTrip to La Union for 2

7. Raffle ticket with incomplete information is null and void.

8. A customer can have multiple entries but can only win once. In case of multiple winnings, customer will get the prize with the highest value.

9. Raffle Draw will be held on February 13, 2023 4:00PM at Ideal Vision Center office (4th floor Brightstar Building, No. 9 Palanza St. corner Silencio St. Brgy: Santol Quezon City) in the presence of a DTI representative.

10. Winners will be awarded with travel voucher and will need to directly coordinate with the official travel agency for all travel details.

11. All travel packages are inclusive of:

  • Roundtrip Airfare for Boracay and Bohol Trip; Land Transfer within La Union
  • 3 Nights Hotel Accommodation
  • Daily Breakfast for 2 persons
  • Full Board Meals: 3 Lunch, 3 Dinner (Food Voucher)
  • Group Tour
  • Use of resorts’ facilities and amenities
  • Travel Insurance

Winners shall be notified via registered mail and email. Name of winners shall be announced and posted at Ideal Vision Center official Facebook page.

12. The prizes are not convertible to cash but transferrable provided that written agreement for transfer is signed by both the winner and organizer.

13. Prizes should be claimed within sixty (60) days from the receipt of registered notice or it will be forfeited in favor of Essilor Philippines and Ideal Vision Center with prior approval of DTI. Prizes validity is 1 year from the date of redemption.

14. Tax (including 20% withholding tax for prizes exceeding P10,000.00) and other expenses that will be incurred in relation to the prize shall be shouldered by the sponsor, Essilor Philippines. Processing of other travel documents and related expenses will be shouldered by the winner.

15. Prizes can be claimed at Ideal Vision Center or affiliated branch nationwide by presenting the winning claim stub with any two (2) valid IDs (Government issued) and their Official Receipt issued by the participating branch.

16. All employees of Ideal Vision Center, Essilor Philippines and participating outlets including their relatives up to second degree of consanguinity or affinity are disqualified from joining the promotion.
